Fatal Notification 2026-08 Machinery Related (Crushed by Spreader)
Initial notification is that a farmer was killed when he was crushed by a spreader whilst disconnecting it from the back of a tractor.
Key Safety Messages:
Ensure that plant and equipment is well maintained and in good order in accordance with the manufacturer’s instructions before using it.
Couple and decouple attachments in line with manufacturer’s instructions.
Ensure that those using machinery are trained and competent to do so.
Before approaching any moving parts, or starting any maintenance task, follow the Safe Stop procedure (handbrake on - controls in neutral - engine off - remove key).
Ensure that maintenance or adjustments are carried out by someone who has the necessary skills, knowledge, and experience to carry out the work safely.
Before carrying out maintenance, ensure that any stored energy is released (for example in springs, compressors or hydraulics) that could cause parts of the machine to move or fall.
Make sure that any parts of the machine or equipment that could fall or move are secured. Don’t work under a machine or load unless you have made it safe.
Have procedures in place for anyone to report damaged or faulty equipment.
